Seek legal advice from an external immigration attorney.

Complete form and attach two original copies of the duly signed with blue ink to be endorsed by the State Department along with a copy of valid passport bio page, the most recent I-94 and visa.
In addition,
If applying for an F1 visa, provide a copy of the I-20.
If applying for an H1B visa, provide the job offer letter and 1797 USCIS notice of receipt of the petition.
If applying for a B1/B2 visa, provide a letter of intent to leave the U.S. describing what you and your dependent will do here, and timeframe of stay.
Note: completed package must be submitted to the US State Department within 30 days of termination from the ÐÓ°ÉÂÛ̳ to be eligible for endorsement.

Drop off the original completed form with HR Operations.

Send the two copies of and the explanation letter to the US State Department for endorsement.
Note: The processing time for State Department is approximately 6-8 weeks.

Notify the staff member when is received, either endorsed or denied.
